Here is the 2012 guest survey. One entry per household. Multiple entries will be disqualified. Must be 18 or older and a U.S. citizen. Gift certificate is not transferrable and may not be redeemed for cash. Certificate must be redeemed by Dec. 1, 2013 or it becomes void. Contest not open to Wisconsin bed and breakfast innkeepers.
